Logic Model Toolkit

Purpose:

  • Identify alignment between the logic model and evaluation plan.
  • Ensure clear ownership of each data collection and analysis.

Directions:

  1. Access your logic model and project plan on Communities360.
  2. Review the outputs, short-term outcomes, and long-term outcomes of your logic model with your sustainability planning and evaluation team. Your outputs and outcomes are the basis for measurement and evaluation of your grant work. They will help you identify which strategy or strategies are having the desired impact and therefore help you identify which strategy or strategies you want to scale up and sustain over time.

Discussion Questions:

  1. To what extent do your logic model and evaluation plan align with each other? What adjustments need to be made to ensure alignment?
  2. Your logic model identifies the measures you will use to determine if each strategy is achieving your desired results. Which measures do you already have access to? Which measures if any need to be created?
  3. Who is responsible for each data collection and analysis? Are these responsibilities reflected in the project plan? In the evaluation plant?
  4. What processes do you have to review data to inform continuous improvement efforts?

Next Steps:

  1. If you have not done so already, complete the Measurement and Evaluation portion of the sustainability self-assessment in A Comprehensive Guide to Sustainability Planning (see page 27).
  2. Using the results of your self-assessment as a guide, strategically integrate questions from pages 9-11 of A Comprehensive Guide to Sustainability Planning into sustainability planning team meetings to reflect on and plan to strengthen your identification of outcomes, plans for formative and summative evaluation, alignment of value to outcomes, assessment of effectiveness, and documentation and communication of outcomes. Identify action steps and integrate them into your project plan.
